Mr. R sustained a fracture to his arm. He required surgery to repair his fractured arm. During the operation, a tube intended for Mr. R’s jugular vein went into his carotid artery and drugs were given through it before it was noticed to be in the wrong place. Mr. R unfortunately died following the injection.
